  • What do I need for my initial appointment?


    In order to make sure your initial appointment gets started on time please bring the following items with you... 

     

    1. Insured's Insurance Card

    2. State Issued ID - Insured and Client

    3. Completed Intake Forms (click here for forms)

     

    * Appointments cancelled less than 24 hours prior to the agreed upon session time will be subject to a $50 fee, and a $60 no call/show fee will be applied to one's account if no notice is given before missing an appointment.
